1) What evidence is there specifically that creatine is safe for long term use?
2) Is there any evidence specifically that creatine does not cause renal damage?
Regarding the first point, there is an excellent study that had 60 subjects in it (40 received 4g/d of CM and 20 were placebo controlled) that lasted a full two years.
The subjects were older and had Parkinson's as creatine and its analogs may have some use in ameliorating neurological conditions but nonetheless, this study correlated a high degree of safety (especially renal parameters which were looked at extensively) and daily creatine monohydrate use at 4g/d x two years continuously.

Regarding the second point, creatine monohydrate may actually IMPROVE renal health. There is a study in which 9 male subjects ages 18-35 a whopping 10g/d of creatine monohydrate x 90 days (yes, I realize the power of this study is not ideal but it "moves the needle" in the right direction).
This study looked at some better/more reliable & specific markers of renal health and it correlated that kidney function & health actually improved slightly in the 9 subjects who ingested the 10g/d of creatine monohydrate for three months.
