If your client crashes after champion select or during the game you get disconnected and you can't go back to the game, this is the problem.
Here's a rioter response on this issue,
1) Make sure you have all available windows updates installed from this website: http://update.microsoft.com
2) Make sure you are an administrator: http://support.microsoft.com/kb/2663817
3) Disable UAC(Windows Vista or Windows 7): http://www.howtogeek.com/howto/windows-vista/disable-user-account-control-uac-the-easy-way-on-windows-vista/
- Be sure to use the "Never notify me when:" setting
4) Run the game as an administrator (For XP users, log in as an administrator):
- Right-Click on the short-cut
- Select the properties tab
- Check the box labeled "Run as administrator"
5) Occasionally, these problems can be caused by a corrupt disk fragment. To check for corruption, run a scandisk on the drive:

6) If all else fails you may need to re-install the game with administrative rights using this guide:
