Previously, we showed evidence that the primary explosion was located within or very near the strongback structure. This implies that there was likely nothing wrong with the rocket itself - but rather, that the rocket was a victim of a strongback explosion.
A comparison of before and after photographs of the strongback, seems to verify this conclusion.
We also present historical cases which may parallel, and shed light upon, the reason this happened. For the most part, NASA has shown a pretty good record of material handling safety. But, there seems to be one pattern of shortcomings, that seems to repeat again and again. The Spacex Falcon-9 AMOS-6 explosion, might have been the latest example in that pattern.
